Previous experience of working in the community Required. Previous knowledge of working with SystemOne would be ideal.
To work as part of the community nursing team in unplanned care ensuring high quality health care is delivered to patients in their own home.
The post holder will be responsible for their own workload and work in collaboration with the wider Community Partnerships and Primary Care Networks.
Experience of Palliative Care/Wound Care/Insulin Administration
